• Patient has a multiple myeloma diagnosis
• Receiving active treatment for their diagnosis at a treatment center in the Houston, TX or San Diego, CA metro area.
• Expected household income for this year is within 300% of the Federal Poverty Level (FPL) —calculate your FPL here
• Application is submitted by a member of the healthcare team where the patient is receiving treatment

MMRC Horizon One (NCT06171685) or MMRC Horizon Two (NCT07053436)

• Patient has a multiple myeloma diagnosis
• Patient is actively participating in MMRC Horizon One (NCT06171685) or MMRC Horizon Two (NCT07053436) in a US state or territory
• Expected household income for 2026 is within 300% of the Federal Poverty Level (FPL) — calculate your FPL here
• Application is submitted by a member of the healthcare team where the patient is receiving treatment or receiving MMRC Horizon One (NCT06171685) or MMRC Horizon Two (NCT07053436)
